I have been trien to fill out my date application but everytime I try it sends me to reset my password and then it won’t send the info I need to my email…called and talked to someone and said that she fixed it but i was at work and I checked when I hit off and it wasn’t in my email spam or junk files…I would really like to get my card cuz I hav already had the dr appointment just hav to do this one last thing…please help

The recommendation I was given did not have the correct prescriber identification number. I have called and sent numerous emails with little assistance. At first the call center didn’t understand the question and kept telling me to “read the email” then when I got through to someone else, I was told I would get a call back and never heard back. Hopefully I’ll hear back today. Still no response to my email asking for an updated letter with the correct number. If a friend had not used this service I would been concerned I just got scammed.
